This form provides for naming your star from the date of birth and time, if available. It is the easy way to decide which star in galaxies of millions of stars is the one you want named after yourself or loved one. Our computer will select a star from the glaxay position which our computer determines is best for that date of birth and time of birth. That name is then assigned that star in our American Star Registry. No one else can be assigned that star, so there is no duplication of stars and names, even though one other person may have the same date of birth as you have.
You may access the specific star which is assigned to you, and the name you choose for it, 24 hours after you pay and recieve the confriming email. All you need do is enter the certificate number. Your star name and all the star data will be brought up on the screen. You can print it from there if you want again. It will remain in our database forever. A copy of the copyright document is registered with the library of Congress, as an official publication.
You may now proceed by filling in the following information and paying the $39.95 registration fee and we will handle it from there. All the information you provide will be incorporated in the original document, so type carefully. Once your payment is acknowledged we proceed with the final phase which is selecting that star and notifying you, and providing you that certificate You may print an immediate copy of the certificate from the email. It will be very nice if you use nice paper.
